[bug-gnutrition] Urgent Task: Fix the Mifflin-St Jeor equation

There is no formally recognized or peer reviewed "midpoint constant" for
the Mifflin-St Jeor equation.  GNUtrition 0.32 and older asked for gender
with its user settings for this very reason.  New releases will need to
be postponed until the male/female options are restored, and the equation
is made usable, so that the results it gets can be reliable and accurate.
